Drug Dose Adjustments (common)
Metformin
Cotrimoxazole
Tenofovir (ARV)
⚠️ Use Cockcroft-Gault CrCl for drug dosing decisions. Use CKD-EPI eGFR for CKD diagnosis and staging. Both equations are estimates and may be inaccurate in extremes of body weight, rapid acute kidney injury, or pregnancy. Always use clinical context to interpret results.

💡 Kidney Health

  • GFR ≥90: Normal or high — but check for other markers of kidney damage
  • GFR 60–89: Mildly decreased — monitor regularly
  • GFR 45–59: Mildly to moderately decreased (CKD Stage 3a)
  • GFR 30–44: Moderately to severely decreased (CKD Stage 3b)
  • GFR 15–29: Severely decreased (CKD Stage 4)
  • GFR <15: Kidney failure (CKD Stage 5)
  • Hypertension and diabetes are the leading causes of CKD in Nigeria
  • Many drugs require dose reduction in renal impairment
